Karakteristik bakteri Klebsiella pneumoniae dan kepekaannya terhadap antibiotik di Badan Rumah Sakit Umum Tabanan tahun 2018 – 2020

Abstract

Background: Antibiotic resistance is a serious global health threat that if the current trend of improper and excessive antibiotic use continues, is predicted to result in 10 million deaths globally by the year 2050. K. pneumoniae bacteria are normal flora that can cause opportunistic infections. This study aims to obtain the characteristics of the K. pneumoniae bacteria and its sensitivity to antibiotics at the Tabanan General Hospital, which is one of the type B referral hospitals and one of the teaching hospitals in Bali, Indonesia.

Methods: This research is a descriptive study with a retrospective approach conducted in May – December 2021. The research location was carried out at Tabanan General Hospital Clinical Laboratory. Samples were taken using a case report form from medical laboratory examination data. Examination data was taken from the results of an automatic antibiotic sensitivity test tool VITEK® 2 system which is carried out routinely at Tabanan General Hospital. Data analysis was carried out descriptively using the Microsoft Excel application for Windows.

Results: From the research, the total number of K. pneumoniae isolates isolated at Tabanan General Hospital in 2018 - 2020 was 109 isolates, K. pneumoniae isolates in 2018 - 2020 were found in blood specimens (29.36%), urine (26.61%), wound bed (20.18%), phlegm (16.51%), tissue (5.50%), and pus (1.83%). The highest sensitivity of K. pneumoniae isolates to the antibiotic amikacin (100%) and the highest resistance to the antibiotic ampicillin (100%).

Conclusion: The majority of K. pneumoniae isolates found at Tabanan General Hospital between 2018 and 2020 came from blood samples. K. pneumoniae isolates showed sensitivity to tigecycline, amikacin, ertapenem, and meropenem; and resistant to ampicillin.

 

Latar Belakang: Resistensi antibiotik merupakan ancaman kesehatan global yang serius dimana jika tren penggunaan antibiotik yang tidak tepat dan berlebihan terus berlanjut, diperkirakan akan mengakibatkan 10 juta kematian secara global pada tahun 2050. Bakteri K. pneumoniae merupakan flora normal yang dapat menyebabkan infeksi oportunistik. Penelitian ini bertujuan untuk memperoleh karakteristik bakteri K. pneumoniae dan kepekaannya terhadap antibiotik di Badan Rumah Sakit Umum (BRSU) Tabanan yang merupakan salah satu rumah sakit rujukan tipe B dan merupakan salah satu rumah sakit pendidikan di Bali, Indonesia.

Metode: Penelitian ini merupakan penelitian deskriptif dengan pendekatan retrospektif yang dilakukan pada bulan Mei – Desember 2021. Tempat penelitian dilakukan di Laboratorium Klinik BRSU Tabanan. Sampel diambil menggunakan case report form dari data hasil pemeriksaan laboratorium medik. Data pemeriksaan diambil dari hasil alat uji kepekaan antibiotik otomatis VITEK® 2 system yang dilakukan secara rutin di BRSU Tabanan. Analisis dilakukan secara deskriptif menggunakan aplikasi Microsoft Excel for Windows.

Hasil: Dari penelitian diperoleh total isolat K. pneumoniae yang diisolasi di BRSU Tabanan tahun 2018 – 2020 sebanyak 109 isolat, isolat K. pneumoniae tahun 2018 – 2020 ditemukan pada spesimen darah (29,36%), urin (26,61%), dasar luka (20,18%), sputum (16,51%), jaringan (5,50%), dan pus (1,83%). Sensitivitas tertinggi isolat K. pneumoniae terhadap antibiotik amikasin (100 %) dan resistensi tertinggi terhadap antibiotik ampisilin (100%).

Simpulan: Mayoritas isolat K. pneumoniae yang ditemukan di BRSU Tabanan periode tahun 2018 hingga 2020 berasal dari sampel darah. Isolat K. pneumoniae menunjukkan sensitivitas terhadap tigesiklin, amikasin, ertapenem, dan meropenem; serta resisten terhadap ampisilin.
