Gambaran kejadian mual muntah pascaoperasi setelah penggunaan opioid pada pasien pascabedah ginekologi di Rumah Sakit Umum Pusat Prof. Dr. I Goesti Ngoerah Gde Ngoerah Denpasar

Abstract

Background: Opioid analgesics, mostly used to treat postoperative pain, can cause various side effects in some patients. One of them is postoperative nausea and vomiting (PONV) which causes discomfort for patients, including patients after gynecological surgery. This research was conducted to describe the incidence of postoperative nausea and vomiting after the use of opioids in post-gynecological surgery patients at Prof. Dr. I Goesti Ngoerah Gde Ngoerah General Hospital, Denpasar.

Method: This study used a descriptive cross-sectional design using primary data through questionnaire interviews and secondary data in the form of medical records for post-gynecological surgery patients who were given post-operative opioids at Prof. Dr. I Goesti Ngoerah Gde Ngoerah Denpasar Denpasar involved 58 patients. The data was analyzed descriptively.

Results: This study showed that 12 patients (20.7%) experienced mild nausea and vomiting and 9 patients (15.5%) experienced moderate nausea and vomiting and 37 patients (63.8%) did not experience postoperative nausea and vomiting. As many as 25 patients (43.1%) with morphine epidurally did not experience PONV, while 8 patients (13.8%) experienced mild nausea and vomiting and 2 patients (3.4%) experienced moderate PONV. Of the total patients given fentanyl, 4 patients (6.9%) did not experience PONV, while 2 patients (3.4%) experienced mild nausea and vomiting and 4 (6.9%) patients experienced nausea and vomiting. currently. While 1 patient with tramadol has no incidence of PONV (1.7%).

Conclusion: There are variations in the distribution of data on incidence rates and categories of nausea and vomiting in post-gynecological surgery patients who were given opioids

 

Latar Belakang: Analgesik opioid, sebagian besar digunakan untuk mengatasi nyeri pascaoperasi, dapat menyebabkan berbagai efek samping pada beberapa pasien. Salah satunya adalah mual muntah pascaoperasi (PONV) yang menimbulkan ketidaknyamanan pasien, termasuk pada pasien pasca bedah ginekologi. Penelitian ini dilakukan untuk menggambarkan kejadian mual muntah pascaoperasi setelah penggunaan opioid pada pasien pascabedah ginekologi di Rumah Sakit Umum Pusat Prof. dr. I Goesti Ngoerah Gde Ngoerah Denpasar.

Metode: Penelitian ini menggunakan desain prospektif cross sectional deskriptif dengan menggunakan data primer melalui wawancara kuisioner dan data sekunder berupa rekam medis pada pasien pascabedah ginekologi yang diberikan opioid pascaoperasi di RSUP Prof. dr. I Goesti Ngoerah Gde Ngoerah Denpasar melibatkan 58 pasien. Kemudian data dianalisis secara deskriptif.  

Hasil: Penelitian ini menunjukkan pasien yang mengalami mual muntah ringan sebanyak 12 (20,7%) dan mual muntah sedang sebanyak 9 pasien (15,5%) dan pasien tidak mengalami mual muntah pascaoperasi yakni sebanyak 37 pasien (63,8%). Pasien yang diberikan morfin secara epidural, tidak mengalami mual muntah adalah 25 pasien (43,1%), sedangkan 8 pasien (13,8%) mengalami mual muntah ringan dan 2 pasien (3,4%) mengalami mual muntah sedang. Dari total pasien yang diberikan fentanyl, pasien yang tidak mengalami mual muntah sebanyak 4 pasien (6,9%), sedangkan sebanyak 2 pasien (3,4%) mengalami mual muntah ringan dan sebanyak 4 (6,9%) pasien mengalami mual muntah sedang. Pada 1 pasien yang diberikan tramadol tidak ditemukan kejadian mual muntah (1,7%). 

Kesimpulan: Terdapat variasi distribusi data angka kejadian maupun kategori mual muntah pada pasien pascabedah ginekologi yang diberikan opioid
