Gambaran cavitating pneumonia sebagai komplikasi dari necrotizing pneumonia: Sebuah serial kasus

Abstract

Introduction: Necrotizing pneumonia is often called cavitating pneumonia, but lung cavities are not always found. Cavitating pneumonia is a radiological picture resulting from an infection in the lung parenchyma, which experiences necrosis, resulting in a cavity forming. Diagnosing necrotizing pneumonia requires analysis of the clinical picture and appropriate radiological results. This case series report discusses the radiological features of cavitating pneumonia as a complication of necrotizing pneumonia

Case Description: There were 4 cases of necrotizing pneumonia with cavitating pneumonia on radiological images. The first case was a ten-year-old child with a history of ALL (acute lymphocytic leukemia). Initial chest radiograph showed multiple thin-walled, regular, partial cavities with surrounding consolidation in the middle zone of the right lung. The second case is an eight-year-old girl with ALL. Serial chest radiographs showed a cavity with consolidation of the cavity with surrounding consolidation in the middle zone of the right lung. The third case was a 59-year-old man with squamous cell carcinoma (SCC) of the larynx. A serial chest examination showed consolidation in the upper to middle zone of the left lung, accompanied by multiple cavities of varying sizes within it. The fourth case is an 85-year-old woman with kidney failure and a history of hepatitis C. Serial chest radiographs showed multiple thin-walled cavities with the largest size of 2.3 x 1.9 cm in the lower zone of the right lung accompanied by consolidation around it.

Conclusion: Cavitating pneumonia is a radiological picture resulting from an infection in the lung parenchyma, which experiences necrosis, resulting in a cavity forming. Cavitating pneumonia in acute conditions can cause significant clinical worsening and can result in respiratory failure, sepsis, and death. Therefore, accuracy is needed in recognizing radiological images.

 

Pendahuluan: Necrotizing pneumonia sering disebut dengan cavitating pneumonia tetapi tidak selalu ditemukan kavitas paru. Cavitating pneumonia adalah gambaran radiologi akibat infeksi yang terjadi pada parenkim paru yang mengalami nekrosis sehingga terbentuk kavitas.  Untuk diagnosis necrotizing pneumonia memerlukan analisis dari gambaran klinis serta hasil radiologis yang tepat. Laporan  serial kasus ini membahas gambaran radiologis cavitating pneumonia sebagai komplikasi dari necrotizing pneumonia

Deskripsi Kasus: Terdapat 4 kasus  necrotizing pneumonia dengan cavitating pneumonia pada gambaran radiologis. Kasus pertama adalah anak usia 10 tahun dengan riwayat ALL (acute lymphocytic leukemia). Pemeriksaan radiografi thorax awal menunjukkan multipel cavitas sebagian berdinding tipis, regular, dengan konsolidasi di sekitarnya pada zona tengah paru kanan.  Kasus kedua adalah anak perempuan usia 8 tahun dengan ALL. Foto thorax serial menunjukkan adanya cavitas dengan konsolidasi cavitas dengan konsolidasi di sekitarnya pada zona tengah paru kanan. Kasus ketiga adalah laki-laki 59 tahun dengan squamous cell carcinoma (SCC) laring. Pemeriksaan thoraks serial menunjukkan konsolidasi pada zona atas hingga tengah paru kiri disertai multipel cavitas dengan ukuran bervariasi di dalamnya. Kasus keempat adalah perempuan usia 85 tahun dengan gagal ginjal dan riwayat hepatitis C. Dari foto thorax serial didapatkan multipel cavitas berdinding tipis dengan ukuran terbesar 2.3 x 1.9 cm pada zona bawah paru kanan disertai konsolidasi di sekitarnya

Simpulan: Cavitating pneumonia adalah gambaran radiologi akibat infeksi yang terjadi pada parenkim paru yang mengalami nekrosis sehingga terbentuk kavitas. Cavitating pneumonia dalam kondisi akut dapat mengakibatkan perburukan klinis yang nyata dan dapat mengakibatkan gagal napas, sepsis serta kematian. Oleh karena itu diperlukan ketepatan dalam mengenali gambaran radiologisnya.
