Temuan insidental mitral annular disjunction: laporan kasus dan tinjauan kepustakaan

Abstract

Introduction: Mitral annular dysjunction (MAD) is a structural abnormality in the mitral valve which can result in various other pathologies such as ventricular arrhythmias and sudden cardiac death.

Case illustration: An incidental finding in a 61 year old woman with mammary carcinoma who underwent trans thoracic echocardiography evaluation, with findings of left atrial dilatation, mild mitral regurgitation, and mitral annular displacement. The patient did not have any significant heart rhythm abnormalities.

Conclusion: MAD is a structural abnormality that is defined as detachment of the continuity of the mitral annulus-left atrial wall with the basal aspect of the posterolateral LV. This is often found in patients with myxomatous mitral valve degeneration and mitral valve prolapse (MVP).

 

Pendahuluan: Mitral annular dysjunction (MAD) merupakan suatu kelainan structural pada katup mitral yang dapat mengakibatkan berbagi patologi lainnya seperti aritmia ventrikel dan kematian jantung mendadak.

Ilustrasi kasus: Sebuah temuan incidental pada wanita usia 61 tahun dengan Karsinoma mamae yang dilakukan evaluasi ekokardiografi trans thorakal, dengan temuan adanya dilatasi atrium kiri, mitral reguritasi ringan, dan mitral annular displacement. Pasien tidak memiliki suatu kelainan irama jantung yang bermakna.

Simpulan: MAD adalah kelainan struktural yang diartikan sebagai pelepasan kontinuitas anulus mitral-dinding atrium kiri dengan aspek basal dari LV posterolateral. Hal ini seringkali dijumpai pada pasien dengan degenerasi katup mitral miksomatus dan mitral valve prolapse (MVP).
