Pengaruh profil lipid terhadap gangguan pendengaran sensorineural pada pasien Diabetes Mellitus Tipe II di Poli Diabetic Centre RSUP Prof. Ngoerah

Abstract

Background: Type 2 diabetes mellitus (T2DM) is a major threat to health with the number of cases of complications from this disease continuing to increase every year. One of them is sensorineural hearing loss which is a microvascular complication in patients with T2DM. However, there is still little research data discussing the lipid profile in DM patients with hearing loss. This study aimed to determine the effect of lipid profiles on sensorineural hearing loss in Type II DM patients at the Diabetic Center of Prof. Hospital. Ngoerah.

Methods: This research method is an observational analytical with a case-control design. This research was conducted from September 2022 to March 2023. The case group was patients with T2DM and hearing loss, while the control group was patients with T2DM only. The research variables were lipid profile (LDL and HDL) as the independent variable, sensorineural hearing loss with type II DM as the dependent variable, and intermediary variables namely age, gender, hypertension, and duration of experiencing DM. The analysis was used to assess the odds ratio (OR) to determine the magnitude of the risk effect for cases compared to controls.

Results: A total of 36 T2DM samples were included in this study, 18 case groups and 18 control samples. Based on the mean age of the sample, 51.14 ± 7.32 years with the majority of the sample being male (60%). Based on the analysis of the lipid profile of case and control sample patients, it was found that there was a significant relationship between the lipid profile of disorders and sensorineural hearing (p<0.05). Based on the risk effect size, high LDL-c and low HDL-c influence the incidence of sensorineural hearing loss with values namely OR 8.45 (95% CI: 1.84-38.75) and OR 5.83 (95% CI: 1.23-27.63).

Conclusion: This study concludes that there is a significant influence of the lipid profile on the incidence of sensorineural hearing loss in T2DM patients at the Diabetic Center of Prof. Hospital. Ngoerah.

 

Latar belakang: Diabetes melitus tipe 2 (DMT2) sudah merupakan salah satu ancaman utama bagi kesehatan dengan jumlah kasus komplikasi dari penyakit ini terus meningkat setiap tahunnya. Salah satunya adalah gangguan pendengaran sensorineural yang merupakan komplikasi mikrovaskuler pada pasien dengan DMT2, namun masih sedikit data penelitian yang membahas profil lipid pada pasien DM dengan gangguan pendengaran. Tujuan penelitian ini adalah untuk mengetahui pengaruh profil lipid terhadap gangguan pendengaran sensorineural pada pasien DMT2 di Poli Diabetic Centre RSUP Prof. Ngoerah.

Metode: Metode penelitian ini adalah observasional analitik dengan rancangan case control. Penelitian ini telah dilakukan dari Bulan September 2022 sampai dengan Maret 2023. Kelompok kasus adalah pasien dengan DMT2 dan mengalami gangguan pendengaran, sedangkan kelompok kontrol adalah pasien hanya dengan DMT2. Variabel penelitian adalah profil lipid (LDL dan HDL) sebagai variabel bebas, Gangguan pendengaran sensorineural dengan DMT2 sebagai variabel terikat, serta variabel perantara yaitu usia, jenis kelamin, hipertensi, dan durasi mengalami DM. Analisis yang digunakan adalah menilai odd ratio (OR) untuk menentukan besar efek risiko kejadian kasus dibandingkan kontrol.

Hasil: Total 36 sampel DMT2 yang termasuk dalam penelitian ini, kelompok kasus yakni 18 sampel dan kontrol 18 sampel. Berdasarkan rerata usia sampel adalah 51,14±7,32 tahun dengan mayoritas sampel berjenis kelamin laki-laki (60%). Berdasarkan pada analisis profil lipid pasien sampel kasus dan kontrol diperoleh adanya hubungan yang signifikan antara profil lipid terhadap gangguan pendengaran sensorineural (p<0,05). Berdasarkan besar efek risiko LDL-c yang tinggi dan HDL-c yang rendah berpengaruh terhadap kejadian gangguan pendengaran sensorineural dengan nilai yaitu OR 8,45 (IK95%: 1,84-38,75) dan OR 5,83 (IK95%: 1,23-27,63).

Simpulan: Penelitian ini menyimpulkan bahwa terdapat pengaruh profil lipid yang bermakna terhadap kejadian gangguan pendengaran sensorineural pada pasien DMT2 di Poli Diabetic Centre RSUP Prof. Ngoerah.
