Studi Kasus: Evaluasi Penggunaan Antibiotik pada Pasien Gangguan Ginjal Akut (GgGA) Akibat Sepsis di Rumah Sakit “X” Jakarta Timur

Abstract

Background: Sepsis is an extreme inflammation that occurs due to an infection that can be life-threatening. Sepsis can cause organ damage, one of which is Acute Kidney Injury (AKI). Administration of antibiotics is a fundamental therapy. Therefore, accuracy in selecting antibiotics, dosage, and administration time is very important. Right patient, indication, dose, time, administration, and documentation must be applied to every treatment. Sepsis-induced acute kidney failure is a critical condition that requires treatment.

Case description: This research is a case report. Dosage, indication, and drug administration become a problem in this case. The dose of imipenem/cilastatin was slightly higher than recommended; there was no indication of moxifloxacin after the sputum culture came out, and the administration of vancomycin for patients with acute kidney failure had to be infusion continuously then after laboratory tests came out on October 19, the dose of vancomycin should be reduced. This study aimed to evaluate the appropriate use of antibiotics in patients with sepsis-induced acute renal failure.

Conclusion: The imipenem/cilastatin dosage is slightly higher than recommended. There is no indication for moxifloxacin after the sputum culture results are out. Administering vancomycin to patients with acute kidney impairment should preferably be done via continuous infusion. Subsequently, after the laboratory test results are available on October 19, the vancomycin dosage should be reduced.

 

Latar belakang: Sepsis merupakan peradangan ekstrem yang terjadi akibat infeksi yang dapat mengancam jiwa. Sepsis dapat menyebabkan kerusakan organ salah satunya adalah GgGA. Pemberian Antibiotik merupakan terapi fundamental. Oleh sebab itu, Ketepatan dalam pemilihan antibiotik, dosis, serta waktu pemberian sangat berperan penting. Tepat pasien, tepat indikasi, tepat dosis, tepat waktu, tepat pemberian dan tepat dokumentasi harus diterapkan pada setiap pengobatan. GgGA yang diinduksi Sepsis merupakan kondisi kritis yang memerlukan penanganan.

Deskripsi kasus: Penelitian ini merupakan sebuah laporan kasus dengan menggunkan metode retrospective. Dosis, indikasi dan pemberian obat menjadi masalah dalam kasus ini. Dosis imipenem/cilastatin sedikit lebih tinggi dari anjuran, tidak ada indikasi moxifloxacin setelah kultur sputum keluar dan pemberian vankomisin untuk pasien gagal ginjal akut harus infus terus menerus kemudian setelah tes laboratorium keluar pada 19 Oktober, dosisnya vankomisin harus dikurangi. Tujuan dari penelitian ini adalah untuk mengevaluasi penggunaan antibiotik yang tepat pada pasien dengan sepsis yang diinduksi GgGA

Kesimpulan: Dosis imipenem/cilastatin sedikit lebih tinggi dari anjuran, tidak ada indikasi moxifloxacin setelah kultur sputum keluar dan pemberian vankomisin untuk pasien gangguan ginjal akut sebaiknya diberikan melalui infus terus menerus kemudian setelah tes laboratorium keluar pada 19 Oktober, dosisnya vankomisin sebaiknya dikurangi.
